Covariation in anomalous correspondence with accommodative vergence

Summary

In a patient with alternating exotropia, the angle of anomaly closely tracked changes in the objective angle when accommodation was stimulated. This suggests a strong link between accommodative convergence and the vergence system in this condition.

Background:

Constant concomitant alternating exotropia is a common form of strabismus.
Understanding the underlying mechanisms of vergence control is crucial for effective treatment.

Observation:

In a patient with exotropia, the angle of anomaly was measured while stimulating accommodative convergence with concave lenses.
The objective angle of exotropia was systematically altered.

Findings:

A perfect correlation (r=1.00) was observed between the angle of anomaly and the objective angle.
The angle of anomaly varied up to 31 prism diopters as the objective angle varied up to 37 prism diopters.

Implications:

This finding highlights the significant role of accommodative convergence in maintaining binocular alignment in certain types of exotropia.
