Clear-cornea cataract extraction in eyes with functioning filtering blebs

R T Oyakawa, A E Maumenee

    American Journal of Ophthalmology

    Summary

Clear-cornea cataract extraction maintained controlled intraocular pressure in most glaucoma patients with filtering blebs. However, some required medication adjustments, and visual outcomes varied.

    Area of Science:

    • Ophthalmology
    • Glaucoma Research
    • Cataract Surgery

    Background:

    • Glaucoma patients often have filtering blebs to manage intraocular pressure (IOP).
    • Cataract extraction is a common procedure that can potentially affect IOP in these patients.

    Purpose of the Study:

    • To evaluate the safety and efficacy of clear-cornea cataract extraction in eyes with pre-existing filtering blebs.
    • To assess the impact of cataract surgery on intraocular pressure control and visual acuity in glaucoma patients.

    Main Methods:

    • Retrospective analysis of 22 glaucomatous eyes with functioning filtering blebs undergoing clear-cornea cataract extraction.
    • Monitoring of intraocular pressure, medication requirements, visual acuity, and intraoperative/postoperative complications.

    Main Results:

    • Intraocular pressure remained controlled (≤22 mm Hg) in 91% of eyes post-surgery.
    • Seven eyes required increased antiglaucoma medication dosage.
    • 16 eyes achieved final visual acuity between 6/6 and 6/9; only 4 had acuity worse than 6/60.

    Conclusions:

    • Clear-cornea cataract extraction is generally safe and effective in maintaining IOP control in glaucoma patients with filtering blebs.
    • While IOP is typically maintained, adjustments in medical therapy may be necessary post-surgery.
    • Visual outcomes are often favorable, but potential complications like bullous keratopathy and vitreous incarceration should be considered.
