Acute hepatitis B: a further serological characterization

J A Payne, D M Jensen, R G Codini

    Journal of Clinical Gastroenterology
    |April 1, 1982
    PubMed
    Summary

Sequential testing for Hepatitis B virus (HBsAg) and related antibodies is crucial for diagnosing acute Hepatitis B infection. Simultaneous absence or presence of viral markers and antibodies can occur during the early recovery phase.

    Area of Science:

    • Hepatology
    • Virology
    • Immunology

    Background:

    • Acute Hepatitis B diagnosis relies on detecting Hepatitis B virus (HBV) antigens and antibodies.
    • The early convalescent period of acute Hepatitis B presents complex serological profiles.
    • Understanding these profiles is key to assessing chronicity and contagion risk.

    Purpose of the Study:

    • To analyze sequential serological markers in patients with acute Hepatitis B.
    • To evaluate the diagnostic utility of simultaneous antigen and antibody detection.
    • To investigate the role of anti-liver specific protein (anti-LSP) in acute Hepatitis B.

    Main Methods:

    • Sequential blood sample analysis from thirteen patients with acute Hepatitis B.
    • Detection of Hepatitis B surface antigen (HBsAg), anti-HBs, HBeAg, anti-HBe, and anti-HBc.

    Related Experiment Videos

  • Monitoring for the presence of anti-liver specific protein (anti-LSP).
  • Main Results:

    • A 'serologic window' (absence of detectable antigens and antibodies) was observed in 9/13 patients.
    • Simultaneous presence of viral antigens and antibodies, suggesting immune complex formation, occurred in 6/13 patients.
    • Anti-LSP was transiently detected in 11/13 patients, with no correlation to viral markers or host antibody titers.

    Conclusions:

    • Sequential serological testing is essential for accurate diagnosis and management of acute Hepatitis B.
    • The dynamic interplay of viral antigens and antibodies in early convalescence impacts etiological assessment.
    • Anti-LSP's role in acute Hepatitis B pathogenesis requires further investigation.
