    Background:

    • A 16-year-old female presented with a gunshot wound to the left upper abdomen.
    • Initial imaging via excretory urography suggested a left kidney was present.

    Observation:

    • Early opacification in the left upper quadrant mimicked a nephrogram on excretory urogram.
    • Surgical exploration revealed the absence of the left kidney.

    Findings:

    • Excretory urography has diagnostic limitations in trauma patients.
    • The initial imaging falsely indicated the presence of a kidney.

    Implications:

    • Alternative diagnostic techniques like radionuclide scanning, arteriography, or tomography are recommended for trauma patients with suspected renal injury.
    • Operative intervention may be necessary for definitive diagnosis.
    • A comprehensive search for ectopic kidneys is advised in relevant cases.
