    Area of Science:

    • Rheumatology
    • Epidemiology
    • Public Health

    Background:

    • Rheumatoid arthritis (RA) is a chronic autoimmune disease impacting joints and potentially other organs.
    • Understanding the long-term mortality risks associated with RA is crucial for patient care and public health strategies.

    Purpose of the Study:

    • To investigate the mortality patterns in a cohort of rheumatoid arthritis patients diagnosed in Stockholm County.
    • To compare the mortality rates of RA patients with the general population and identify specific causes of excess mortality.

    Main Methods:

    • A cohort study design was employed, following 1165 RA patients diagnosed in 1971 in Stockholm County.
    • Mortality data were collected through 1978 and compared against the general Stockholm population's mortality rates.

    Main Results:

    • RA patients exhibited approximately 2.5 times higher mortality than the general population.
    • Excess mortality was most pronounced in females and linked to infections, gastrointestinal, urogenital, and rheumatic diseases.
    • Cancer mortality rates were comparable to the general population, with no specific cancer types showing excess risk.
    • Rheumatoid arthritis was listed as an underlying or contributory cause of death in 38% of deceased patients.

    Conclusions:

    • Rheumatoid arthritis is associated with a substantial increase in overall mortality.
    • Infections and disease-related complications are significant contributors to excess mortality in RA patients.
    • RA management should prioritize addressing comorbidities and infection risks to improve patient survival.
