    This study reports a rare case of central retinal artery thrombosis associated with unusual empty sella and Sylvian aqueduct stenosis. These combined anomalies offer new insights into complex neurovascular and sellar pathologies.

    Background:

    • Central retinal artery thrombosis is a critical condition affecting vision.
    • Empty sella and Sylvian aqueduct stenosis are rare sellar and cerebrospinal fluid pathway abnormalities, respectively.

    Observation:

    • A patient presented with central retinal artery thrombosis.
    • Radiological imaging revealed an enlarged sella turcica and ventricular dilatation.
    • Neuroradiological examination identified Sylvian aqueduct stenosis and an uncommon empty sella variant.

    Findings:

    • The empty sella was characterized by intrasellar persistence of the recessus infundibuli and an intrasellar arachnoidocele.
    • This specific combination of intrasellar anomalies, particularly the persistence of the recessus infundibuli with an arachnoidocele, has not been previously reported.
