    Background:

    • Polycythemia vera (PCV) is a myeloproliferative neoplasm characterized by increased red blood cell mass.
    • Patients with PCV often have altered platelet function, increasing bleeding risk.
    • Pre-operative management of PCV patients requires careful consideration of bleeding risks.

    Observation:

    • A 54-year-old woman with clinically asymptomatic PCV underwent elective eyelid surgery.
    • Following an uncomplicated surgical procedure, the patient ingested a small dose of aspirin.
    • Within 24 hours, she developed a severe bleeding diathesis.

    Findings:

    • Aspirin, a common antiplatelet agent, exacerbated the underlying bleeding tendency in this PCV patient.
    • Salicylates significantly impair platelet aggregation, particularly in patients with pre-existing platelet dysfunction.
    • The patient's bleeding disorder was successfully managed with appropriate interventions.

    Implications:

    • This case underscores the critical importance of assessing bleeding risks in PCV patients, even when asymptomatic.
    • Physicians should exercise caution when prescribing aspirin or other salicylates to PCV patients, especially perioperatively.
    • Further research into the specific mechanisms of salicylate-induced bleeding in PCV is warranted.
