Background:

Basal cell epitheliomas, particularly pigmented and nodular types, often present as widespread lesions.
Treatment options traditionally include surgery and radiation therapy, which may not be ideal for extensive cases.

Observation:

A single patient with multiple pigmented basal cell epitheliomas on the back was treated with topical 5% fluorouracil cream daily.
The treatment duration was nine months.

Findings:

Complete eradication of all nodules, papules, and plaques of pigmented basal cell epitheliomas was achieved.
Topical chemotherapy demonstrated efficacy in treating superficial epitheliomas.

Implications:

Topical 5% fluorouracil cream presents a viable alternative to surgery and radiation for widespread nodular superficial epitheliomas.
Prolonged treatment and diligent follow-up are crucial for successful outcomes in managing extensive basal cell epitheliomas.
