Adrenal adenoma. Isolated testosterone secretion

T R Kelly, D J Mayors, P S Boutsicaris

    The American Surgeon
    |November 1, 1982
    PubMed
    Summary

A rare adrenal adenoma caused virilization through isolated testosterone secretion, bypassing typical 17-ketosteroid production. Surgical removal of the adenoma successfully resolved the patient's hirsutism.

    Area of Science:

    • Endocrinology
    • Oncology

    Background:

    • Adrenal adenomas can cause hormonal imbalances.
    • Virilization in women is often linked to androgen excess.

    Observation:

    • A patient presented with virilization and hypertestosteronemia.
    • Urinary 17-ketosteroid levels were normal, leading to an initial misdiagnosis of an ovarian disorder.

    Findings:

    • The adrenal adenoma exclusively secreted testosterone.
    • A unique enzymatic pathway in the adenoma converted androstenedione directly to testosterone, bypassing 17-ketosteroid metabolites.

    Implications:

    • This case highlights a rare mechanism of hormone production in adrenal adenomas.
    • Accurate diagnosis of androgen excess requires comprehensive evaluation beyond standard urinary markers.
    • Surgical intervention for the specific adrenal adenoma provided a complete cure for hirsutism.
