The renal glomerulus in hypoxic cor pulmonale

J L Campbell, P M Calverley, D Lamb

    Thorax
    |August 1, 1982

    Summary

Kidney glomerular size was larger in patients with chronic bronchitis and emphysema, correlating with arterial oxygen levels. These kidney changes may be reversible with oxygen therapy.

    Area of Science:

    • Nephrology
    • Pulmonology
    • Pathology

    Background:

    • Chronic bronchitis and emphysema, often termed "blue and bloated", can impact renal function.
    • Understanding kidney structural changes in these patients is crucial for managing cardiorenal complications.

    Purpose of the Study:

    • To investigate kidney glomerular size in patients with chronic bronchitis and emphysema.
    • To determine the relationship between glomerular size, oxygenation, and cor pulmonale.
    • To explore the potential reversibility of renal structural changes with oxygen treatment.

    Main Methods:

    • Postmortem kidney sections from 16 chronic bronchitis/emphysema patients and 7 controls were analyzed.
    • A custom semi-automatic technique measured mean capillary tuft area.
    • Glomerular size was correlated with arterial oxygen tension (Po2) and cor pulmonale indices.

    Main Results:

    • Glomerular tuft area was significantly larger in bronchitis/emphysema patients compared to controls.
    • Glomerular size correlated with arterial Po2, especially when accounting for oxygen therapy.
    • Increased glomerular size was associated with a higher percentage of identifiable juxtaglomerular apparatus.

    Conclusions:

    • Severe chronic bronchitis and emphysema are associated with enlarged glomeruli.
    • These renal structural changes may be linked to altered salt and water handling.
    • Oxygen therapy might potentially reverse these kidney alterations.
