    Background:

    • Rheumatoid arthritis (RA) is a systemic autoimmune disease typically affecting multiple joints.
    • Monolateral presentation of RA is rare and poses diagnostic challenges.

    Observation:

    • A case study details a patient with left monolateral rheumatoid arthritis.
    • This patient had a history of ischemic cerebral lesion nine years prior, causing right-sided hemiparesis.

    Findings:

    • Clinically and radiologically, the rheumatoid arthritis appeared strictly unilateral.
    • Histological examination, however, indicated that RA lesions are inherently bilateral.
    • The induced rest period following the stroke may have contributed to articular protection.

    Implications:

    • This case supports the hypothesis explaining rare unilateral manifestations of rheumatoid arthritis.
    • Periods of immobility, such as those following a stroke, might offer a protective effect on joints.
    • Further research is warranted to explore the mechanisms behind unilateral RA presentation and joint protection.
