    Background:

    • Neurological manifestations of sarcoidosis affect 5% of patients, with cranial nerve involvement being common.
    • The eighth cranial nerve is the fourth most frequently affected cranial nerve in neurosarcoidosis.
    • Granulomatous meningitis is the presumed central nervous system lesion in neurosarcoidosis.

    Observation:

    • Sarcoidosis is an underrecognized cause of fluctuating sensorineural hearing loss.
    • Hearing fluctuation can be the sole presenting symptom of neurosarcoidosis.
    • While fluctuating hearing loss has been noted, this is the first reported case where it was the initial complaint.

    • Diagnosis of neurosarcoidosis involves chest radiography, eye examination, and potentially lymph node biopsy.
    • Hilar adenopathy and uveitis can suggest the diagnosis.
    • Hearing loss in sarcoidosis typically improves with steroid therapy.

    • Otolaryngologists should consider sarcoidosis in the differential diagnosis of fluctuating hearing loss.
    • This highlights the diverse neurological presentations of sarcoidosis.
    • Prompt diagnosis and management can lead to hearing recovery.
