    Background:

    • Three adult patients presented with severe respiratory embarrassment due to functional upper airway obstruction.
    • Objective criteria confirmed the diagnosis in all cases, necessitating tracheostomy.

    Findings:

    • Systemic evaluations ruled out organic etiologies for the airway obstruction.
    • Psychiatric evaluations revealed underlying psychological disorders in each patient.

    Implications:

    • Functional airway obstruction should be considered in the differential diagnosis of adult airway obstruction.
    • This condition highlights the interplay between psychological factors and severe respiratory compromise.
