Appendicitis is an acute inflammatory condition of the vermiform appendix, most commonly caused by obstruction of its lumen.     Background:

    • Pelvic actinomycosis is a rare, chronic granulomatous infection caused by Actinomyces species.
    • Intrauterine contraceptive devices (IUDs) are a known risk factor for pelvic actinomycosis.
    • The clinical presentation often mimics pelvic malignancy, leading to diagnostic challenges.

    Observation:

    • A case of pelvic and rectal actinomycosis is presented in a patient with an intrauterine contraceptive device.
    • The condition mimicked an indolent, noninfectious process resembling pelvic cancer.
    • Suppurative disease persisted even after the removal of the intrauterine device.

    Findings:

    • Preoperative diagnosis of pelvic actinomycosis was not suspected in this case.
    • Actinomycosis diagnosis requires a high index of suspicion in IUD users presenting with specific symptoms.
    • Pathogenic actinomyces can be found in the genital tract of IUD users.

    Implications:

    • Abdominal pain, recurrent vaginal bleeding, or adnexal masses in IUD users warrant a thorough investigation for actinomycosis.
    • Prompt identification and treatment of genital tract actinomycosis are essential to prevent disease progression.
    • Increased awareness among clinicians regarding pelvic actinomycosis in IUD users is crucial for timely diagnosis and management.
