    Summary

Choledochoduodenostomy is an effective treatment for common bile duct stones when the duct diameter exceeds 1.2 cm. This procedure offers a safe alternative to T-tube insertion, managing existing and preventing future complications of choledocholithiasis.

    Area of Science:

    • Gastroenterology and Hepatobiliary Surgery
    • Surgical Management of Biliary Calculi

    Background:

    • Choledocholithiasis, or common bile duct stones, presents significant management challenges.
    • Existing therapeutic options carry risks and may not address all potential complications.

    Purpose of the Study:

    • To evaluate the efficacy and safety of choledochoduodenostomy in managing choledocholithiasis.
    • To assess its role as a therapeutic and prophylactic measure for common bile duct stone complications.

    Main Methods:

    • Retrospective analysis of 200 cases undergoing choledochoduodenostomy.
    • Focus on cases with calculus-containing common bile ducts measuring 1.2 cm in internal diameter.

    Main Results:

    • Choledochoduodenostomy demonstrated excellent therapeutic and prophylactic outcomes.
    • Mortality rates were comparable to those associated with T-tube insertion.
    • The procedure effectively bypassed distal obstructive factors, including residual stones and stenosis.

    Conclusions:

    • Choledochoduodenostomy is a highly recommended procedure for specific cases of choledocholithiasis.
    • It provides a safe and effective solution for managing current and preventing future complications.
    • Advantages include bypassing distal obstructions, addressing recurrent stones, and managing biliary sludge.
