Colonic "urticaria' pattern due to early ischemia

    Summary

    Colonic urticaria, a bleblike pattern on colon mucosa, is not always allergic. This study found it caused by colonic distention and submucosal edema, potentially indicating early ischemic changes.

    Background:

    • The radiographic appearance termed "colonic urticaria" presents as unusual bleblike mounds on the colon mucosal surface.
    • This pattern has been historically associated with allergic reactions.

    Observation:

    • Three patients presented with the colonic urticaria pattern without any evidence of an allergic state.
    • The observed pattern in all patients was directly attributed to colonic distention.

    Findings:

    • Histopathological examination revealed the pattern to be solely submucosal edema.
    • No allergic or inflammatory cellular infiltration was identified.

    Implications:

    • The findings suggest that colonic distention-induced submucosal edema can mimic colonic urticaria.
    • This presentation may represent early ischemic changes in the colonic mucosa due to distention, warranting further investigation into the pathophysiology of this radiographic finding.
