    Background:

    • Photosensitivity reactions can be triggered by various exogenous agents.

    Observation:

    • A patient developed persistent photosensitivity affecting the face, neck, and hands after using an after-shave lotion containing musk ambrette.
    • The skin eruption persisted for over three years despite treatment.
    • The patient exhibited minimal erythema to ultraviolet B and strong photosensitivity to ultraviolet A when exposed to musk ambrette.

    Findings:

    • The patient demonstrated clear signs of persistent photosensitivity induced by musk ambrette.
    • Phototesting confirmed ultraviolet A hypersensitivity to both the chemical and the product.
    • The clinical presentation and photobiological response strongly suggest persistent light reactivity.

    Implications:

    • Musk ambrette should be used with caution due to its potential to induce persistent photosensitivity.
    • Dermatologists should consider musk ambrette as a potential cause in patients with unexplained persistent photosensitivity.
    • Further research into the mechanisms of persistent light reactivity induced by fragrance ingredients is warranted.
