Progressive renal failure in surgically treated hyperparathyroidism

J Dixon, J F Smith

    Journal of Clinical Pathology
    |July 1, 1981
    PubMed
    Summary

This study reviewed 22 hyperparathyroidism cases, finding renal failure was common. Correcting hypercalcemia prevented progressive renal failure from nephrocalcinosis.

    Area of Science:

    • Nephrology
    • Endocrinology
    • Pathology

    Background:

    • Hyperparathyroidism can lead to severe complications, including renal failure.
    • Understanding the causes of mortality in hyperparathyroidism is crucial for patient management.

    Purpose of the Study:

    • To review pathological findings in patients who died after hyperparathyroidism surgery.
    • To compare these findings with cases of untreated hyperparathyroidism.

    Main Methods:

    • Retrospective review of necropsy findings in 22 patients (18 post-surgery, 4 untreated).
    • Analysis of causes of death, focusing on renal failure and its association with hyperparathyroidism.

    Main Results:

    • 14 out of 22 patients experienced renal failure at death.
    • Nephrocalcinosis was a significant cause of renal failure, particularly in post-operative patients with persistent hypercalcemia.
    • Tertiary hyperparathyroidism was noted in cases with chronic glomerulonephritis, cystinuria, and polycystic disease.

    Conclusions:

    • Renal failure is a major concern in hyperparathyroidism, both treated and untreated.
    • Effective management of hypercalcemia is key to preventing progressive renal failure due to nephrocalcinosis.
    • Hyperparathyroidism can be secondary to underlying renal disease.
