    Background:

    • Rheumatoid vasculitis is a severe complication of rheumatoid arthritis.
    • Treatment options for rheumatoid vasculitis are limited and often associated with significant side effects.

    Purpose of the Study:

    • To evaluate the efficacy of plasma exchange (PE) in treating patients with rheumatoid vasculitis.
    • To determine the optimal duration and adjunctive therapies for PE in rheumatoid vasculitis.

    Main Methods:

    • Nine patients with rheumatoid vasculitis were treated with plasma exchange.
    • Treatment protocols varied in duration and combination with immunosuppressive agents like cyclophosphamide, methylprednisolone, and azathioprine.

    Main Results:

    • Plasma exchange arrested new lesion development in 7 out of 9 patients.
    • Prolonged PE, with or without cyclophosphamide and methylprednisolone, led to healing of existing lesions without relapse.
    • Short-term PE, with or without azathioprine, resulted in treatment failure and early relapses.
    • Decreased levels of IgG rheumatoid factor and anticomplementary activity correlated with clinical improvement.

    • Combination therapy with cyclophosphamide and methylprednisolone may enhance PE efficacy.
    • Treatment duration and choice of adjunctive immunosuppression are critical factors in achieving sustained remission.
