    Summary

Cerebral thromboangiitis obliterans (CTAO) is presented in a young woman with gangrene and neurological symptoms. Findings suggest a link between cerebral and peripheral thromboangiitis obliterans.

    Area of Science:

    • Neurology
    • Vascular Medicine
    • Pathology

    Background:

    • The existence of cerebral thromboangiitis obliterans (CTAO) remains debated in medical literature.
    • Thromboangiitis obliterans (TAO), also known as Buerger's disease, primarily affects peripheral arteries.

    Observation:

    • A case report details a young woman presenting with recurrent thrombophlebitis, digital gangrene, and bilateral anterior opercular syndrome (Foix-Chavany-Marie).
    • Cerebral angiography revealed significant stenosis in the fronto-opercular branches of both middle cerebral arteries.
    • Histopathological examination of digital arteries showed adventitial fibrosis, lumen narrowing/occlusion, and lymphocytic infiltrates, with occasional phlebitis in veins.

    Findings:

    • The clinical presentation, laboratory results, and angiographic findings in this patient are consistent with CTAO.
    • Histological evidence supports an inflammatory occlusive process in both peripheral and cerebral vessels.
    • The observed features suggest a potential etiological relationship between cerebral and peripheral thromboangiitis obliterans.

    Implications:

    • This case provides evidence supporting the existence of CTAO as a distinct clinical entity.
    • Understanding the link between peripheral TAO and CTAO may improve diagnostic and therapeutic strategies.
    • Further research is warranted to elucidate the pathogenesis and prevalence of CTAO.
