    Summary

Tuberculous alcoholic patients can safely receive pyrazinamide-containing regimens, even with initial liver issues. Liver function improved with treatment, showing good tolerance and supporting the use of these drugs with careful monitoring.

    Area of Science:

    • Hepatology
    • Infectious Diseases
    • Pharmacology

    Background:

    • Tuberculosis (TB) treatment in alcoholic patients presents challenges due to potential co-existing liver damage.
    • Pyrazinamide is a crucial component of TB regimens, but its hepatotoxicity is a concern, especially in vulnerable populations.

    Purpose of the Study:

    • To evaluate the safety and efficacy of pyrazinamide-containing regimens in alcoholic patients with tuberculosis.
    • To assess the impact of chemotherapy on liver function in this patient group.

    Main Methods:

    • A prospective study involving alcoholic tuberculous patients treated with a pyrazinamide-containing regimen (IRSZ).
    • Liver histology and function tests were assessed at baseline and after two months of treatment.
    • A control group received a non-pyrazinamide regimen (IRSE), and a separate group received pyrazinamide (Z) alone.

    Main Results:

    • Significant improvements in liver histology and function tests were observed after two months of pyrazinamide-containing chemotherapy.
    • Both pyrazinamide-containing and control regimens demonstrated good liver tolerance.
    • Pyrazinamide monotherapy also showed excellent liver tolerance in nonalcoholic patients.

    Conclusions:

    • Pyrazinamide-containing regimens are well-tolerated in tuberculous alcoholic patients, provided there is no significant pre-existing hepatic dysfunction.
    • Careful monitoring of liver function, including SGPT and bilirubin levels, is recommended during treatment.
