Epidural morphine for postoperative pain relief

S Reiz, J Ahlin, B Ahrenfeldt

    Acta Anaesthesiologica Scandinavica
    |April 1, 1981

    Summary

Epidural morphine provided superior pain relief and longer duration after hip surgery compared to intramuscular administration. While intramuscular offered faster onset, epidural morphine required lower total doses with fewer side effects.

    Area of Science:

    • Anesthesiology
    • Pharmacology
    • Orthopedic Surgery

    Background:

    • Postoperative pain management is crucial following hip surgery.
    • Morphine is a common analgesic, but its administration route impacts efficacy and side effects.

    Purpose of the Study:

    • To compare the analgesic potency, duration, and side effects of epidural versus intramuscular morphine.
    • To evaluate morphine administration for hip surgery pain control.

    Main Methods:

    • Thirty-three patients undergoing hip surgery were randomized into two groups.
    • Epidural morphine (2 mg preservative-free) was compared to intramuscular morphine (10 mg).
    • Pain relief, duration, total dose, and side effects were monitored over 15 hours.

    Main Results:

    Related Experiment Videos

    • Intramuscular morphine had a faster onset of action.
    • Epidural morphine demonstrated significantly higher quality of pain relief and longer duration.
    • Lower total morphine doses were required with the epidural route (3.6 mg vs. 41 mg).
    • Epidural morphine had fewer and milder side effects, notably less nausea/vomiting (20% vs. 55%) and urinary retention (20%).

    Conclusions:

    • Epidural morphine offers superior and longer-lasting analgesia for hip surgery compared to intramuscular administration.
    • Preservative-free solutions are recommended for epidural morphine to minimize side effects.
    • Close patient monitoring is advised due to potential delayed respiratory depression with epidural opioids.
