    Background:

    • Cysticercus cellulosae, the larval stage of Taenia solium (pork tapeworm), can cause rare intraocular infestations.
    • Ocular cysticercosis presents a diagnostic and therapeutic challenge due to its potential to affect various ocular structures.

    Purpose of the Study:

    • To report a case of bilateral multifocal intraocular cysticercosis.
    • To describe the surgical management and outcomes of this rare condition.

    Main Methods:

    • Pars plana vitrectomy for intravitreal cysticercus removal in the left eye.
    • Sclerotomy and scleral buckling for subretinal cysticercus removal with retinal detachment in the right eye.
    • Incidental removal of a subconjunctival cysticercus.

    Main Results:

    • Successful surgical removal of intravitreal and subretinal cysticercus in the left and right eyes, respectively.
    • Management of a rhegmatogenous retinal detachment secondary to subretinal cysticercus.
    • One peripapillary subretinal cyst could not be removed.

    Conclusions:

    • Surgical intervention, including vitrectomy and scleral buckling, can be effective in managing intraocular cysticercosis.
    • Complete eradication of all parasites may not always be feasible.
    • Prompt surgical management can lead to favorable outcomes in ocular cysticercosis.
