    Summary

This study presents an abdomino-cervical esophagectomy without thoracotomy for esophageal cancer. This minimally invasive technique reconstructs the food canal using a stomach tube with a low mortality rate.

    Area of Science:

    • Surgical Oncology
    • Gastrointestinal Surgery
    • Thoracic Surgery

    Context:

    • Esophageal cancer resection often requires extensive surgery.
    • Minimally invasive approaches are sought to reduce morbidity.
    • Achieving clear margins is crucial for successful cancer treatment.

    Purpose:

    • To describe a technique for esophagectomy without thoracotomy.
    • To evaluate the feasibility and outcomes of this surgical approach.

    Summary:

    • The procedure involves abdomino-cervical blunt dissection for total esophagectomy, removing the proximal stomach and associated lymph nodes.
    • A stomach tube is reconstructed from the greater curvature and advanced to the neck for anastomosis.
    • The technique leverages the craniocaudal blood supply of the esophagus for easier reconstruction.

    Impact:

    • This abdomino-cervical esophagectomy technique demonstrates a low mortality rate (15% in 20 cases).
    • It offers a potentially less invasive alternative for esophageal cancer resection.
    • Successful reconstruction facilitates restoration of the food pathway.
