    Summary

    Chronic open-angle glaucoma significantly reduces corneal sensitivity due to optic nerve damage. This study links reduced sensitivity to long ciliary nerve damage, supporting pressure atrophy as the cause of glaucomatous optic nerve damage.

    Context:

    • Chronic open-angle glaucoma (COAG) is a leading cause of irreversible blindness.
    • Optic nerve damage is a hallmark of glaucoma, often associated with elevated intraocular pressure (IOP).
    • Corneal sensitivity changes in glaucoma patients are not fully understood.

    Purpose:

    • To investigate the relationship between corneal sensitivity and optic nerve status in patients with COAG.
    • To explore the potential link between reduced corneal sensitivity and damage to ocular nerves.

    Summary:

    • Corneal sensitivity was assessed in 91 eyes with COAG using an electronic-optical hand esthesiometer.
    • Eyes exhibiting glaucomatous optic nerve cupping or atrophy showed significantly increased threshold values (reduced sensitivity) compared to those with normal optic discs.
    • No direct correlation was found between intraocular pressure (IOP) at the time of measurement and the duration of COAG.
    • Reduced corneal sensitivity is attributed to damage of the long ciliary nerves located between the choroid and sclera.

    Impact:

    • Findings suggest that glaucomatous optic nerve atrophy may result from primary pressure-induced atrophy of the retina.
    • Highlights the potential for corneal sensitivity testing as an indicator of glaucomatous damage.
