The internship: some disquieting findings

J D Lindy, B L Green, M Patrick

    The American Journal of Psychiatry
    |January 1, 1980

    Summary

Psychiatric residents without an internship showed better outpatient psychotherapy skills than those with one. This suggests internships may negatively impact empathy development in psychiatric training.

    Area of Science:

    • Psychiatry
    • Medical Education
    • Psychotherapy Training

    Background:

    • A national natural experiment in psychiatric education occurred between 1970-1976.
    • Residents were divided into two groups: those who completed an internship and those who did not.

    Purpose of the Study:

    • To compare the initial two-year training performance of psychiatry residents with and without prior internships.
    • To identify potential differences in skill development, particularly in outpatient psychotherapy.

    Main Methods:

    • Compared 22 psychiatry residents with an internship against 25 without one.
    • Evaluated faculty ratings on responsibility, insight, teachability, knowledge, and performance in various psychiatric settings (inpatient, consultation, emergency, outpatient).

    Main Results:

    • Initial faculty evaluations for responsibility, insight, teachability, and knowledge were similar between groups.
    • Overall performance in inpatient, consultation, and emergency psychiatry was comparable.
    • Residents without an internship significantly outperformed those with an internship in outpatient psychotherapy ratings.

    Conclusions:

    • The internship may be associated with negative effects on empathy development.
    • The findings raise concerns about the impact of traditional internship structures on specific psychiatric training outcomes.
    • Further research is needed to understand the mechanisms behind these observed differences.
