Reflex anuria from unilateral ureteral obstruction

J D Hull, S Kumar, P G Pletka

    The Journal of Urology

    Summary

Severe pain from a left ureteral stone caused reflex anuria, a previously undescribed condition. Spontaneous stone passage resolved the anuria, highlighting a unique pain-induced urinary response.

    Area of Science:

    • Nephrology
    • Urology
    • Pathophysiology

    Background:

    • Ureteral stones are a common cause of renal colic and can lead to urinary obstruction.
    • Anuria, the absence of urine output, typically indicates severe kidney dysfunction or obstruction.

    Observation:

    • A 20-year-old woman presented with severe pain and anuria lasting 18 hours.
    • A calcium oxalate stone was identified impacted in her distal left ureter.

    Findings:

    • The patient experienced spontaneous passage of the ureteral stone.
    • Following stone passage, normal urine output resumed.
    • Excretory urography revealed no abnormalities in the right kidney.

    Implications:

    • This case suggests a novel mechanism of reflex anuria triggered by severe ureteral stone pain.
    • The findings propose that intense pain may initiate this unusual syndrome, distinct from direct obstruction.
    • Understanding this reflex anuria is crucial for diagnosing and managing complex urinary stone cases.
