    Background:

    • Ovarian pregnancy is a rare form of ectopic pregnancy, accounting for approximately 1-3% of all ectopic pregnancies.
    • Twin pregnancies are common, but when combined with ovarian implantation, they represent an exceptionally rare clinical scenario.

    Observation:

    • This report presents a case of ovarian twin pregnancy.
    • A review of the literature identified three previously reported cases, totaling four cases worldwide.

    Findings:

    • All four reported cases of ovarian twin pregnancy occurred in parous women.
    • The affected women were all in their thirties.

    Implications:

    • The findings suggest that multiparous women in their thirties may represent a demographic with a potentially higher predisposition to ovarian twin pregnancies.
    • Further research is needed to understand the underlying mechanisms and risk factors associated with this rare condition.
