    Summary

    A novel myocardial support system demonstrated remarkable functional recovery and ultrastructural preservation in canine hearts during hypothermic cardiopulmonary bypass. This new cardioplegia method effectively prevented ischemia, unlike control hearts.

    Background:

    • Myocardial preservation during cardiopulmonary bypass is critical.

    Purpose of the Study:

    • To evaluate a new myocardial support system for cardioplegia.
    • To assess the efficacy of continuous infusion cardioplegia during hypothermic cardiopulmonary bypass.
    • To investigate myocardial functional and ultrastructural recovery.

    Main Methods:

    • A novel myocardial support system was employed in 12 dogs.
    • Cardioplegia was induced via continuous aortic root infusion of potassium chloride (with or without propranolol).
    • Total body hypothermia (20°C), controlled perfusion, and specific blood gas/electrolyte management were utilized.

    Main Results:

    • Remarkable functional recovery to normal was observed post-perfusion.
    • Ultrastructural preservation of the subendocardium was significant after 4 hours.
    • Control hearts (spontaneous fibrillation) exhibited severe damage.

    Conclusions:

    • The new myocardial support system and cardioplegia protocol provide excellent myocardial protection.
    • This method effectively prevents ischemia and preserves myocardial integrity during extended hypothermic bypass.
    • It offers a promising alternative to conventional cardioplegia techniques.
