    Background:

    • Conjunctivitis follicularis presents with cellular infiltrates, often leading to assumptions of allergic causes.
    • The presence of lymphocytes, eosinophils, and mast cells suggests a potential allergic etiology.

    Observation:

    • Three cases of conjunctivitis follicularis were studied using immunological methods.
    • Immunoglobulin levels in lacrimal fluid (IgG, IgA, IgM, IgE) were quantified.
    • Immunofluorescence detected immunoglobulin-producing plasma cells in conjunctival biopsy specimens.

    Findings:

    • Conjunctival infiltrates showed a predominance of plasma cells producing IgA and IgM.
    • Plasma cells producing IgG and IgE were found infrequently.
    • Quantitative immunoglobulin analysis in lacrimal fluid corroborated these findings.

    Implications:

    • The results challenge the common assumption of an allergic, IgE-mediated pathomechanism in conjunctivitis follicularis.
    • Further research is needed to elucidate the non-allergic causes of this condition.
    • This study provides a basis for re-evaluating diagnostic and treatment strategies for conjunctivitis follicularis.
