    Background:

    • Intravenous urography (IVU) is associated with a persistent, albeit low, fatality rate.
    • The exact cause of sudden fatalities during IVU remains poorly understood.
    • Arrhythmogenic effects of urographic contrast agents require thorough investigation.

    Purpose of the Study:

    • To assess the arrhythmogenic effects of a commonly used urographic contrast agent.
    • To identify patient demographics and pre-existing conditions associated with contrast-induced arrhythmias.

    Main Methods:

    • A clinical trial involving 58 patients undergoing IVU.
    • Standard bolus injection techniques were employed for contrast agent administration.
    • Electrocardiogram (ECG) monitoring was used to detect post-injection arrhythmias.

    Main Results:

    • 12 out of 58 patients (20%) developed post-injection arrhythmias.
    • Seven patients (12%) experienced significant or potentially serious arrhythmias, including paroxysmal tachycardias and atrioventricular block.
    • Arrhythmias were more frequent in older males; abnormal baseline ECGs were not predisposing factors.

    Conclusions:

    • Urographic contrast agents can induce significant cardiac arrhythmias.
    • Older males appear to be at higher risk, potentially due to latent coronary heart disease.
    • Minor side effects of contrast injection did not correlate with arrhythmia development.
