    Background:

    • Physical urticarias are hypersensitivity reactions to physical stimuli.

    Observation:

    • A patient presented with small weals on extremities during cold weather, specifically in the vicinity of cold contact, not at the site of contact.
    • Lesions were seasonal, occurring only during colder months.
    • No significant laboratory abnormalities, including histamine or eosinophil chemotactic factor (ECF) levels, were detected in the venous effluent of affected areas.

    Findings:

    • Skin reactivity to acetylcholine was normal.
    • Reduced skin reactivity to histamine and compound 48/80 was observed.
    • The clinical presentation suggests a localized form of physical urticaria.

    Implications:

    • This case may represent a localized variant of the rare cold reflex urticaria.
    • Further research is needed to elucidate the specific mechanisms of this localized physical urticaria.
    • This presentation highlights the importance of considering atypical physical urticaria presentations in clinical practice.
