A rare scleral abscess, initially misdiagnosed as episcleritis or metastatic cancer, was successfully treated in a breast cancer patient. Prompt antibiotic and surgical intervention led to uncomplicated recovery.

Area of Science:

  • Ophthalmology
  • Infectious Diseases

Background:

  • A 47-year-old woman with a history of breast cancer presented with a focal epibulbar mass.
  • The mass was initially diagnosed as nodular episcleritis but did not respond to corticosteroid treatment.

Observation:

  • An excisional biopsy was performed to rule out epibulbar metastatic carcinoma.
  • The biopsy revealed a localized staphylococcal abscess that had eroded the sclera and exposed the uveal tract.

Findings:

  • The patient underwent surgical repair with a scleral patch graft.
  • Treatment included oral amoxicillin/clavulanic acid and topical cefazolin.

Implications:

  • This case highlights infectious scleral abscess as an uncommon entity.

  • Differential diagnoses for epibulbar masses should include infectious causes, alongside nodular episcleritis and metastatic tumors.
