Summary

Hyskon (dextran-70) may cause pulmonary edema by increasing lung fluid and protein leakage. This study in dogs suggests a non-cardiogenic mechanism for Hyskon-induced pulmonary edema.

Background:

  • Hyskon (dextran-70) is used for hysteroscopy to improve uterine visualization.
  • Pulmonary edema of unknown origin has been reported in patients post-hysteroscopy.
  • The mechanism of Hyskon-induced pulmonary edema requires investigation.

Purpose of the Study:

  • To determine if Hyskon causes cardiogenic or non-cardiogenic pulmonary edema.
  • To investigate the effect of Hyskon on pulmonary microvascular permeability using a canine model.

Main Methods:

  • Dogs were infused with Hyskon or whole blood to maintain specific left ventricular end-diastolic pressure.
  • Bronchoalveolar lavage (BAL) and plasma protein levels were measured.
  • Pulmonary microvascular membrane permeability was assessed by the BAL protein to plasma protein ratio.

Main Results:

  • Dogs receiving Hyskon showed increased protein in BAL fluid post-infusion.
  • The ratio of BAL protein to plasma protein was significantly higher in the Hyskon group compared to the blood group.
  • Hyskon administration altered pulmonary microvascular membrane permeability.

Conclusions:

  • Hyskon can cause pulmonary edema through increased leakage of plasma proteins into the alveoli.
  • The study suggests a significant non-cardiogenic component in Hyskon-induced pulmonary edema.
  • Findings indicate Hyskon may contribute to the observed pulmonary complications in patients.
