Background:

  • Vascular density in benign prostatic hyperplasia (BPH) remains uncharacterized.
  • Previous research indicated prostatic carcinoma has twice the vessel density of normal prostate tissue.

Purpose of the Study:

  • To characterize the vascular density of benign prostatic hyperplasia (BPH) tissue.
  • To compare vessel density across different histological components within BPH.

Main Methods:

  • Analysis of 15 benign prostatic hyperplasia (BPH) cases from open prostatectomy specimens.
  • Immunohistochemical staining for Factor VIII-related antigen to identify vessels.
  • Computer-assisted image analysis for quantitative measurement of vessel density (vv/mm²).

  • Mean vessel density in transition zone tissue was 70.2 vv/mm².
  • Epithelial hyperplastic nodules showed high vessel density (mean 99.3 vv/mm²), comparable to prostate carcinoma.
  • Hypercellular stromal nodules also exhibited significantly higher vessel density (mean 64.7 vv/mm²) than adjacent stromal areas (mean 36.5 vv/mm²).

Conclusions:

  • Vascular density in benign prostatic hyperplasia (BPH) is higher than previously reported for benign prostate tissue.
  • Nodular morphology in BPH, both epithelial and stromal, is associated with significantly increased vascularity.
  • Elevated vascular density in BPH, especially in nodular regions, warrants further investigation into its clinical implications.
