Summary

This study compared three methods for measuring tacrolimus levels in transplant patients. Liver transplant patients with dysfunction showed higher tacrolimus levels due to cross-reacting metabolites, unlike kidney transplant patients.

Area of Science:

  • Pharmacology
  • Transplantation Medicine
  • Analytical Chemistry

Background:

  • Tacrolimus is a crucial immunosuppressant post-transplantation.
  • Accurate monitoring of tacrolimus blood and plasma concentrations is vital for patient outcomes.
  • Assay methods can be influenced by patient-specific factors like liver function.

Purpose of the Study:

  • To compare three analytical methods for determining tacrolimus trough concentrations.
  • To investigate the impact of liver dysfunction on tacrolimus measurements.
  • To identify potential interferences in tacrolimus assays.

Main Methods:

  • Solid-phase extraction (SPE) with ELISA.
  • Liquid-liquid extraction (LLE) with methylene chloride (MC) and ELISA.

Related Experiment Videos

  • High-performance liquid chromatography (HPLC) fractionation followed by ELISA.
  • Main Results:

    • Trough plasma tacrolimus concentrations varied from 0.1 to 5.2 ng/ml.
    • Liver transplant patients with dysfunction showed higher plasma tacrolimus by SPE-ELISA and MC-ELISA compared to HPLC-ELISA.
    • Cross-reacting tacrolimus metabolites were detected in liver dysfunction patients but not kidney transplant patients.

    Conclusions:

    • Assay methodology impacts tacrolimus measurement, especially in liver transplant patients with dysfunction.
    • Tacrolimus metabolites can interfere with ELISA-based assays in patients with impaired liver function.
    • Blood tacrolimus concentrations are significantly higher than plasma concentrations.
