Summary

Critically ill patients with liver failure need prompt nutritional support. Protein intake should be carefully managed, especially during hepatic encephalopathy, and resumed quickly to prevent malnutrition.

Area of Science:

  • Hepatology
  • Critical Care Medicine
  • Nutritional Support

Background:

  • Critically ill patients with liver failure experience catabolism, necessitating prompt caloric support.
  • Hepatic encephalopathy presents unique diagnostic and management challenges in this patient population.

Purpose of the Study:

  • To provide an overview of nutritional management strategies for critically ill patients with liver failure.
  • To outline the appropriate use of protein in patients with hepatic encephalopathy.

Main Methods:

  • Literature review and synthesis of current clinical guidelines.
  • Analysis of patient cases with varying degrees of liver failure and hepatic encephalopathy.

Main Results:

  • Nitrogen products should be withheld only in cases of overt hepatic coma while precipitating factors are addressed.
  • Protein reintroduction should be rapid to mitigate protein deprivation consequences.

Conclusions:

  • Nutritional support is crucial for catabolic critically ill patients with liver failure.
  • Protein management requires careful consideration during hepatic encephalopathy but should be resumed promptly.
  • Post-recovery, cirrhotic patients' energy and protein needs normalize.
