Interdisciplinary collaboration: strengthening documentation

The JCAHO's "Agenda for Change" initiative contains new and revised standards for acute care facilities. To meet these, nursing departments will have to reevaluate current forms and documentation to ensure that collaborative interdisciplinary practice is demonstrated. Development and use of one such form is described.
