Background:

Antrochoanal polyps (ACPs) are maxillary sinus polyps extending into the choana.
Their pathogenesis, particularly the relationship with allergy, remains unclear.

Purpose of the Study:

To compare the histologic features of ACPs with allergic and non-allergic nasal polyps.
To elucidate the pathogenesis of ACPs.

Main Methods:

Histologic examination of 40 ACP cases.
Comparison with histologic findings of allergic and non-allergic nasal polyps.

Main Results:

No allergy was detected in ACP patients.
ACPs exhibited more severe inflammatory cell infiltration but less eosinophilic infiltration compared to allergic polyps.

Submucous glands were less pronounced in ACPs, which were often of the fibrous type.

Conclusions:

ACPs have minimal causal relationship with nasal allergy.
Pathogenesis likely involves edematous hypertrophy of respiratory epithelium due to inflammation, rather than glandular distension.
