Background:

Small cell osteosarcoma is an exceptionally rare primary bone malignancy, accounting for only 1-4% of all osteosarcoma cases.
Distinguishing small cell osteosarcoma from other small round blue cell tumors, such as Ewing sarcoma, is critical for appropriate treatment.
The calcaneus represents an unusual anatomical site for osteosarcoma development.

Observation:

A unique case of small cell osteosarcoma was identified in the calcaneus of an 8-year-old female patient.
Histopathological examination revealed a tumor composed of small round cells with morphological similarities to Ewing sarcoma.
Foci of lacy osteoid formation were observed interspersed among the tumor cells.

Findings:

The tumor's histological features, including small round cell morphology and osteoid production, are characteristic of small cell osteosarcoma.
The calcaneal location is atypical for this type of bone sarcoma, making initial diagnosis potentially challenging.
Differential diagnosis must carefully consider other small round blue cell tumors that can occur in bone.

Implications:

This case underscores the importance of considering rare diagnoses in pediatric bone tumors, even with unusual presentations.
Accurate histological assessment and consideration of tumor location are crucial for correct diagnosis and effective management of small cell osteosarcoma.
Further research into the specific characteristics and treatment of small cell osteosarcoma in pediatric patients may improve outcomes.
