Summary

Enhanced macrophage activity, indicated by high tumour necrosis factor-alpha (TNF-alpha) and neopterin levels, is linked to a worse prognosis in alcoholic hepatitis patients. These markers may predict mortality in this liver disease.

Background:

  • Chronic alcoholic liver disease involves immune dysregulation, including altered T-cell function and cytokine profiles.
  • Elevated levels of tumour necrosis factor-alpha (TNF-alpha) and other cytokines suggest enhanced macrophage activity.
  • Protracted macrophage activation and TNF-alpha production may exacerbate alcoholic hepatitis and influence disease progression.

Purpose of the Study:

  • To investigate the relationship between serum cytokine and neopterin levels and prognosis in alcoholic hepatitis.
  • To determine if specific immunological markers can predict mortality in patients with alcoholic hepatitis.

Main Methods:

  • Serum levels of TNF-alpha, interleukin-1 beta (IL-1 beta), gamma-interferon, and neopterin were sequentially measured in 43 alcoholic hepatitis patients and 20 controls.

    • Serum TNF-alpha and neopterin levels serve as significant independent predictors of mortality in alcoholic hepatitis.
    • These immunological markers may aid in risk stratification and management of alcoholic hepatitis.
