Summary

Massive chronic intervillositis (MCI), a rare placental lesion, was linked to recurrent pregnancy loss in a case study. This finding suggests MCI may be an identifiable cause of recurrent spontaneous abortions.

Background:

  • Massive chronic intervillositis (MCI) is an uncommon placental lesion.
  • MCI is typically associated with poor fetal growth and adverse pregnancy outcomes.
  • Previously, MCI was not linked to spontaneous abortion or recurrent pregnancy loss.

Observation:

  • This study details a patient experiencing 10 spontaneous abortions.
  • Massive chronic intervillositis was documented in 4 of 5 pregnancies across all trimesters.
  • Histology revealed extensive infiltration of the intervillous space by inflammatory cells and fibrin, without chronic villitis.

Findings:

  • The cellular infiltrate comprised primarily monocyte/macrophage populations (LCA+, CD68+, occasional CD45RO+).
  • Elevated maternal serum alpha-fetoprotein levels were noted in two pregnancies.
