Hypothyroidism in association with systemic amyloidosis

Summary

Systemic amyloidosis can cause hypothyroidism, even with thyroid gland infiltration. Patients with amyloid goiter require screening for thyroid dysfunction and awareness of replacement therapy challenges.

Background:

Systemic amyloidosis involves amyloid protein infiltration of organs, frequently affecting the thyroid gland.
Thyroid dysfunction is uncommon despite frequent thyroid gland amyloid infiltration in systemic amyloidosis.

Observation:

A case of hypothyroidism is presented in a patient with amyloid goiter.
Histologic and serologic evaluations did not reveal alternative causes for hypothyroidism.
Hormonal assays indicated progressive hypothyroidism resistant to oral and parenteral replacement therapy.

Findings:

Amyloid infiltration of the thyroid gland can lead to hypothyroidism.
Literature review confirms other reported cases of thyroid dysfunction in systemic amyloidosis.

Implications:

Amyloid goiter necessitates screening for thyroid dysfunction in systemic amyloidosis patients.
Physicians must recognize potential difficulties in thyroxine replacement therapy for hypothyroidism in these patients.
