Summary

Hepatitis C virus (HCV) infection is detectable in patients after bone marrow transplant (BMT). Most patients with confirmed anti-HCV antibodies showed liver damage, but BMT immunosuppression did not hinder antibody production.

Background:

  • Bone marrow transplantation (BMT) is a critical treatment for hematologic malignancies and aplastic anemia.
  • Hepatitis C virus (HCV) infection poses a significant health concern, particularly in immunocompromised patient populations.
  • Assessing HCV seroprevalence and clinical impact post-BMT is crucial for patient management.

Purpose of the Study:

  • To determine the prevalence of hepatitis C antibodies (anti-HCV) in patients who underwent BMT.
  • To investigate the clinical significance of HCV infection, including liver damage, in this cohort.
  • To evaluate the impact of BMT-associated immunosuppression on HCV antibody production.

Main Methods:

  • Eighty-five patients undergoing BMT were tested for anti-HCV using second-generation enzyme-linked immunosorbent assay (ELISA).
  • Positive ELISA results were confirmed with a supplemental second-generation recombinant immunoblot assay (RIBA-4).
  • Liver function tests were performed to assess for biochemical evidence of liver damage.

Main Results:

  • 9.4% of patients were initially anti-HCV positive by ELISA, with 87.5% confirmed by RIBA-4.
  • Six of seven confirmed positive patients exhibited biochemical liver damage, though no cases of fulminant hepatitis or hepatic failure occurred.
  • Liver enzyme abnormalities were significantly more common in seropositive patients (p < 0.05).
  • Antibody production against HCV was not significantly affected by BMT immunosuppression.
  • Antibodies to core antigen (C22) may be detected earlier in infection, and ELISA preceded RIBA-4 in some cases.

Conclusions:

  • HCV infection is present in a notable proportion of BMT recipients.
  • HCV infection post-BMT is associated with liver damage but generally not severe hepatic complications.
  • BMT immunosuppression does not abolish the host's ability to mount an antibody response to HCV.
  • Specific antibody patterns and assay timing may offer insights into early HCV detection.
