Summary

Macroenzymes, or high molecular mass serum enzymes, are often found with advanced age, cardiovascular disease, malignancy, and rheumatologic conditions. Awareness of macroenzymes can prevent unnecessary medical procedures.

Background:

  • Macroenzymes are serum enzymes with higher molecular mass than normal.

Purpose of the Study:

  • To retrospectively investigate the prevalence of specific diagnoses in patients with detected macroenzymes.

  • Retrospective review of medical records of 42 patients with macroenzymes (macro-creatine kinase, macro-lactate dehydrogenase, macro-aspartate aminotransferase, macroamylase) identified between 1986 and 1990.
  • Analysis of patient demographics and associated diagnoses.

Main Results:

  • Common findings included advanced age (except for macro-aspartate aminotransferase), cardiovascular disease, malignant lesions (especially with macro-creatine kinase), and rheumatologic disease (with macro-lactate dehydrogenase).
  • Macroenzyme disorders present as isolated, persistent elevations in total serum enzyme levels.

Conclusions:

  • Macroenzymes are frequently associated with advanced age and specific conditions like malignancy and rheumatologic disease.
  • Physicians should recognize macroenzymes to avoid unnecessary diagnostic procedures for elevated serum enzyme levels.
