Summary

Girls with central precocious puberty often have enlarged ovaries. Treatment with GnRH analogue and growth hormone can lead to very large ovaries with a polycystic appearance after cessation.

Area of Science:

  • Pediatric Endocrinology
  • Reproductive Endocrinology
  • Medical Imaging

Background:

  • Ovarian ultrasonography is crucial for diagnosing precocious puberty.
  • Ovarian appearance influences treatment decisions in girls with sexual precocity.

Purpose of the Study:

  • To retrospectively analyze ovarian volume and polycystic ovarian appearance in girls with sexual precocity using ultrasound.
  • To compare these findings across different subtypes and treatment groups of precocious puberty.

Main Methods:

  • Retrospective analysis of ultrasound examinations in girls with sexual precocity.
  • Categorization into groups: untreated CPP, treated CPP (GnRH analogue +/- GH), post-treatment, premature thelarche, and premature adrenarche.
  • Calculation of ovarian volume and assessment for polycystic appearance, with comparison to control data using statistical analysis.

Main Results:

  • All studied groups showed larger ovarian volumes (SD scores) than controls.
  • Girls who stopped GnRH analogue and GH treatment had significantly larger ovarian volumes and higher prevalence of polycystic ovarian appearance (83%) compared to those on GnRH analogue alone.
  • Premature adrenarche group had smaller ovarian volume SD scores than untreated central precocious puberty.

Conclusions:

  • Central precocious puberty is associated with persistently large ovaries.
  • Combined GnRH analogue and GH treatment leads to very large ovaries with increased polycystic appearance upon cessation.
  • Central precocious puberty and its treatment contribute to a higher prevalence of polycystic ovarian appearance.
