Is skin preparation necessary before insulin injection?

V A Koivisto, P Felig

    Summary

Routine skin preparation with isopropyl alcohol significantly reduces bacteria on injection sites for diabetic patients. However, omitting this step before insulin injections did not lead to infections in over 1700 instances.

    Area of Science:

    • Endocrinology
    • Microbiology
    • Dermatology

    Background:

    • Diabetes management often involves frequent insulin injections.
    • Skin preparation is a standard practice to prevent infection at injection sites.
    • The necessity of routine skin preparation before insulin injections requires further investigation.

    Purpose of the Study:

    • To evaluate the impact of routine skin preparation on bacterial flora.
    • To assess the infection risk associated with omitting skin preparation before insulin injections in diabetic patients.

    Main Methods:

    • Thirteen insulin-dependent diabetic patients participated in the study.
    • Skin bacterial counts were measured before and after 70% isopropyl alcohol cleansing.
    • Patients alternated between preparing and not preparing the skin before insulin injections over 3-5 months.

    Main Results:

    • A 5-second cleanse with 70% isopropyl alcohol reduced bacterial counts by 82-91%.
    • Over 1700 insulin injections were administered without skin preparation.
    • No local or systemic infections were observed in any of the participants.

    Conclusions:

    • Routine skin preparation with alcohol significantly reduces skin bacteria.
    • Pre-injection skin preparation may not be essential for preventing infections in insulin-treated diabetic patients.
