Uterine receptivity in an oocyte donation programme

Summary

Oocyte donation recipients with a multilayered endometrium and uterine artery pulsatility index (PI) below 3.4 had higher conception rates. A PI below 3.4 is crucial for successful embryo implantation.

Background:

Oocyte donation offers a unique model to study embryo implantation factors.
Embryos from younger oocytes are expected to yield higher implantation rates.

Purpose of the Study:

To evaluate transvaginal sonographic markers of endometrial receptivity in oocyte donation recipients.
To identify sonographic predictors of successful embryo implantation.

Main Methods:

Retrospective analysis of 117 fresh embryo transfers in 99 women undergoing oocyte donation.
Sonographic assessment of endometrial thickness, echogenic pattern, and uterine artery pulsatility index (PI) and resistance index (RI).

Main Results:

A multilayered echogenic endometrial pattern was significantly more prevalent in conception cycles (91.2%) versus non-conception cycles (44.4%).
No significant differences in endometrial thickness, PI, or RI were found between conception and non-conception groups.
No pregnancies occurred when the uterine artery PI was 3.4 or higher (sensitivity 100%).

Conclusions:

A multilayered echogenic endometrium and a uterine artery PI below 3.4 are favorable predictors of successful implantation in oocyte donation.
Delaying embryo transfer until these sonographic criteria are met may improve pregnancy outcomes.
