Summary

Trabeculectomy with laser suture lysis significantly reduces intraocular pressure in glaucoma patients. This glaucoma surgery demonstrates a higher success rate and fewer complications compared to trabeculectomy alone.

Background:

  • Glaucoma, a leading cause of irreversible blindness, necessitates effective surgical interventions to manage intraocular pressure (IOP).
  • Trabeculectomy is a common glaucoma surgery, but its efficacy can be modulated by scleral flap management.

Purpose of the Study:

  • To evaluate the safety and efficacy of trabeculectomy with laser suture lysis in patients with open-angle or angle-closure glaucoma.
  • To compare outcomes of trabeculectomy with laser suture lysis versus standard trabeculectomy.

Main Methods:

  • A cohort of 49 eyes (45 patients) underwent trabeculectomy with a tightly sutured scleral flap, with 41 eyes subsequently receiving laser suture lysis (experimental group).
  • The experimental group was compared to a control group of 41 eyes (39 patients) who underwent trabeculectomy without laser suture lysis.
  • Intraocular pressure and incidence of ocular complications were monitored over a mean follow-up period of 14.7 months.

Main Results:

  • The experimental group achieved a significantly lower mean post-operative IOP (1.96 kPa / 14.7 mmHg) compared to pre-operative levels (5.93 kPa / 44.5 mmHg).
  • The IOP control rate was higher in the experimental group (89.8%) compared to the control group (73.2%).
  • The incidence of ocular complications, including shallow anterior chamber, choroid detachment, hyphema, and lens opacity, was significantly lower in the experimental group.

Conclusions:

  • Trabeculectomy combined with subsequent laser suture lysis is a safe and effective surgical approach for managing glaucoma.
  • Laser suture lysis enhances the success rate of trabeculectomy while reducing the risk of postoperative ocular complications.
