Summary

Propionibacterium acnes caused a rare subacute lung infection in a patient with chronic lung disease. This case highlights a new pulmonary pathogen, Propionibacterium acnes, in interstitial fibrosis.

  • A 65-year-old male with chronic lung disease exacerbation presented with subacute respiratory symptoms.
  • Open lung biopsy revealed interstitial fibrosis.
  • Gram stain showed numerous bacteria, later identified as Propionibacterium acnes.

Findings:

  • Propionibacterium acnes was isolated in pure culture from lung tissue.
  • Macrophages within the biopsy stained positive for Propionibacterium acnes antigen.
